Acknowledgement of Terms and Conditions
It is the responsibility of the customer to contact the District and request modifications to their water budget through a Commercial Water Budget Modification Form. An approved budget modification will become effective starting with the bill following the date the District processes the Commercial Water Budget Modification Form. An approved request will increase the base water budget (Tier 1 and/or Tier 2) and will be determined on a case-by-case basis. No retroactive modifications will be made for increases in employees or change in business. Any customers providing falsified information to the District may be liable for back charges. Bill calculations based on falsified information will be recalculated with corrected customer account details.
I affirm that the information contained herein, including attachments, is complete and accurate. I understand that all water budget moficiations are subject to change based on future water conservation requirements.
