Reason for Fireflow Test Request

Fireflows will be conducted utilizing a hydraulic model on open zones; closed zones will be conducted by a flowtest in the field.

Please provide the following information:

Point "A" indicates hydrant to be flowed for GPM

Point "B" indicates hydrant measuring Static/Residual PSI

Please attach a map of the two hydrants to be tested, and mark hydrant "A" and "B".
